We're seeking a compassionate, detail-oriented Licensed Practical Nurse (LPN) to oversee the medical care of residents in our group homes. This is a Monday through Friday position with no evenings, weekends, or holidays required.

What You'll Do:
• Use your clinical knowledge to advocate for our residents within the healthcare system.
• Conduct resident assessments, infection control evaluations, and medication administration observations to ensure quality care and regulatory compliance.
• Oversee the medical care of individuals in group home settings, ensuring their health needs are met.
• Teach medication administration classes to direct care staff and ensure compliance with medication protocols.
• Provide medical guidance and triage in coordination with the RN, including participation in shared on-call rotation (phone triage only).
• Travel within a 4-county area to visit homes and support staff.

What We’re Looking For:
• Active MN LPN license in good standing.
• Computer proficiency and comfort with electronic documentation.
• Experience in community health, residential settings, or similar environments a plus.
• Excellent communication and organizational skills.
• A desire to make a difference in the lives of others in a meaningful, long-term way.
