Position Summary
The Registered Dietitian is responsible for assessing the nutritional stus of PACE participants and developing individualized nutrition interventions based on clinical da, medical conditions, and participant preferences. This role serves as an active member of the Interdisciplinary Team (IDT) and plays a key role in care planning, participant educion, and ongoing reassessment to ensure optimal health outcomes.
Responsibilities
Schedule / Shift:

• Monday through Friday, Business Hours

Key Duties:

• Conduct comprehensive nutritional assessments upon enrollment and as needed, ensuring compliance with PACE regulions.
• Develop, implement, and upde individualized care plans in collaborion with the interdisciplinary team (IDT).
• Provide nutrition counseling and educion to participants, families, and caregivers to support overall health and disease management.
• Monitor participants nutritional stus, including weight, hydrion, dietary intake, swallowing issues, and potential drug-nutrient interactions.
• Maintain accure documention and communice changes in participants condition to the IDT in a timely manner, ensuring coordined care.

Requirement
Educion:

• Required: Bachelors degree in Food, Nutrition, or Dietetics from an accredited institution

Licenses & Certificions:

• Required: Registered Dietitian (RD) credential through the Commission on Dietetic Registrion (CDR)
• NJ: Must be registered or eligible for registrion with the CDR (Commission on Dietetic Registrion)
• Additional Certificion: ServSafe certificion preferred

Skills:

• Minimum 1 year of experience working with the elderly populion (training provided if not present)
• 2+ years of clinical nutrition experience preferred
• Strong knowledge of medical nutrition therapy, disease management (e.g., diabetes, renal, cardiac), and nutritional assessment techniques.
• Ability to interpret labs, evalue dietary intake, identify risks (malnutrition, dehydrion), and make evidence-based recommendions.
