Registered Dietitian

FULL TIME Hybrid role. 1 week in facilities of central valley California 3 weeks remote monthly. Supports multiple skilled nursing facilities in the Central Valley area, while maintaining where you live. Travel reimbursement allocated.

The Registered Dietitian (RD) is responsible for providing evidencebased medical nutrition therapy to residents living in a skilled nursing facility (SNF). This role ensures compliance with federal and state regulations, promotes optimal nutritional status, and supports residentcentered care through collaboration with the interdisciplinary team.

Essential Duties and Responsibilities

• Complete nutrition assessments, reassessments, and care plans per CMS, Title 22 and facility guidelines

• Provide medical nutrition therapy for residents with complex medical conditions

• Monitor weight trends, intake, labs, and nutritionrelated risks; recommend interventions as needed

• Ensure compliance with federal, state, and facility regulations and guidelines

• Accurately document assessments and contribute to MDS and care planning processes

• Collaborate with the interdisciplinary team to support residentcentered care

• Participate in quality improvement activities related to nutrition and hydration

• Provide nutrition education to residents, families, and staff as appropriate

Qualifications

• Bachelors degree or higher in Nutrition or Dietetics

• Registered Dietitian Nutritionist (RDN) credential through the Commission on Dietetic Registration

• Knowledge of CMS regulations and longterm care standards, prefer understanding of California state regulation Title 22

• SNF experience preferred but not required
