Under the supervision of a manager or supervisor, delivers comprehensive nutrition services to clients, patients, and eligible community members. Ensures seamless referrals to breastfeeding support, healthcare providers, insurance resources, and community services in a client/patient-centered environment. Provides specialized, high risk nutrition services developing individualized care plans and collaborating with multidisciplinary healthcare teams to optimize outcomes. Serves as key nutrition resource for staff, offering expertise, guidance, and training as needed. Develops and conducts group nutrition education sessions and delivers engaging, evidence-based presentations to community organizations and partners throughout Ingham County.

  1. Conducts comprehensive assessments of medical and nutrition histories; interprets findings to identify client/patient needs, priorities, and concerns. Provides individualized client-centered counseling and education to promote optimal health and nutrition outcomes.
  2. Engages clients/patients in collaborative goal setting by reviewing assessment findings, evaluating comprehension, assessing readiness for change, and developing personalized nutrition care plans to support behavior modification.
  3. Determines and issues appropriate food packages tailored to individual medical, nutritional, and cultural needs and preferences, ensuring client comprehension of available foods, participating stores, and how to access WIC benefits.
  4. May maintain an assigned caseload and assume a leadership role in case management across multiple public health programs, including but not limited to MIHP, WIC, and Community Health Centers. Collaborates with interdisciplinary teams and accepts referrals from other programs to ensure coordinated, client-centered nutrition assessment, counseling, and follow-up services.
  5. Demonstrates diplomacy and professionalism in managing complex client interactions; applies conflict resolution and negotiation skills to achieve positive outcomes, even in situations where client requests cannot be met.
  6. Exemplifies strong cross-cultural communication skills, effectively interacting with clients, families, vendors, community partners, and staff at all levels. Utilizes translation and interpretation services as needed in person, by phone, or via telehealth/video conferencing.
  7. Provides high-risk nutrition counseling, developing, implementing, and monitoring individualized care plans for clients with complex medical or nutritional needs.
  8. Ensures follow-up and continuity of care, evaluating progress toward nutrition goals and adjusting care plans accordingly.
  9. Coordinates and facilitates referrals, maintaining continuity of care and supporting appropriate follow-up with healthcare providers and community resources.
  10. Manages specialized nutrition requests, including special food packages and formulas, in accordance with physician orders, clinical guidelines, and program standards.
  11. Maintains current knowledge and competencies in nutrition science and best practices; actively participates in required trainings and professional development related to nutrition and breastfeeding.
  12. Develops and updates educational materials, lesson plans, and nutrition education resources to support effective client and community education.
  13. Serves as a subject matter expert in nutrition, providing technical consultation and support to clinic staff, healthcare professionals, and community organization.
  14. Collaborates with multidisciplinary healthcare teams, including physicians and other providers, to coordinate care for clients with nutrition-related diagnoses (e.g., failure to thrive) and to obtain authorizations for specialized nutrition products.
  15. Completes advanced training in breastfeeding management and integrates learned techniques and evidence-based practices into client-centered breastfeeding support, promotion, and education.
  16. Participates as a member of interdisciplinary care teams, contributing nutrition expertise to the development and management of comprehensive care plans.
  17. Serves as a preceptor and mentor for dietetic interns, nursing students and medical residents; provides training and evaluation input.
  18. Ensures compliance with confidentiality and documentation standards, maintaining secure and accurate clinical records in accordance with WIC and Health Department policies and procedures.
  19. Conducts home and hospital visits as needed, delivering nutrition counseling and support to County residents in a variety of care settings.
  20. Engages in professional development activities including workshops, conferences, and other continuing education opportunities to maintain Registered Dietitian credentialing and uphold professional excellence.
Other Functions :
  • Performs other duties as assigned.
  • Must adhere to departmental standards in regard to HIPAA and other privacy issues.
  • During a public health emergency, the employee may be required to perform duties similar to, but not limited, to those in his/her job description.

Education: Possession of Registration as a Dietitian by the Commission on Dietetic Registration. Possession of a Masters Degree if RD obtained after 1/1/2024.

Experience: Two years of professional experience as a Registered Dietitian (RD), preferably in a hospital or public health setting.

Demonstrates effective communication skills by engaging, interacting and working with all individuals to ensure clarity and understanding in all interactions.

  • This position is expected to maintain reliable transportation for travel between sites, to home visits, community events and trainings which at times includes overnight state-wide travel to MDHHS trainings and conferences.
  • This position transports equipment to satellite clinics and outreach events.
  • This position performs light work requiring the ability to exert 20 pounds or less of force in the physical requirements above.
  • This position primarily requires close visual acuity to perform tasks within arms reach such as: viewing a computer screen, using measurement devices, inspecting and assembling parts, etc.
  • This position requires the ability to communicate and respond to complex and/or technical information to professionals, colleagues and others in a clear and effective manner both in person and over the phone.
  • This position requires the ability to operate a PC/laptop, be proficient in Microsoft Word and to learn and become proficient in multiple records and data systems.
  • This position requires the ability to handle varying and often high levels of stress.

Working Conditions:

  • This position works in both an indoor and outdoor environment. While indoors, there is no planned exposure to weather conditions, however exposure to noise, odors, and widely varying temperatures is common. While outdoors, exposure to lights, noises, odors, temperatures or weather conditions cannot be controlled and may be extreme.
  • This position is exposed to communicable diseases, blood, other body fluids, etc.

  • This position is required to travel for meetings and appointments. Some appointments may be held at personal residences where levels of cleanliness and safety vary.
