Position Summary
At Stony Brook Medicine, our Clinical Educator Diabetes provides state-of-the-art diabetes self-management education (DSME), and Medical Nutrition Therapy (MNT) to patients and their families/care-partners, helping to optimize their diabetes management. Conducts nutrition assessments. Collaborates with the Program Coordinator in the assessment, planning, implementation, and evaluation of the patient-centered diabetes education program at Stony Brook Medicine offices.
Duties of Clinical Educator Diabetes may include but are not limited to:
- Evaluate and assesses patients' management and education needs, self-care behaviors, and obstacles to the successful implementation of therapy in a variety of settings.
- Assesses nutritional needs and provides individual nutrition counseling in an outpatient setting.
- Provides individual formal training on diabetes management and risk factor reduction in an outpatient setting (SMBG, CGM, medication management education including injectable ones, CSII)
- Provides MNT and/or DSME for Diabetes in pregnancy, GDM, and post-partum.
- Maintains and documents in patient records per standard of care and to meet requirements for certification.
- Responsible for communicating with referring physician.
- Collaborates with the Program Coordinator in the instruction of group classes.
- Collaborates with the Program Coordinator in the DSME program design and overall success and maintenance of certification
- Collaborates with the Program Coordinator in review and assessment of program resources.
- Collaborates with the Program Coordinator in documenting and processing medical records, billing, and statistical records according to Stony Brook Medicine Policy and Procedures.
- Is an active member of the Diabetes Advisory Committee.
- Maintains professional registration and certification and state licensure. Attends continuing education programs, conferences to maintain CDCES and RD or RN credentials.
Qualifications
Required:
- Certified Diabetes Care and Education Specialist (CDCES).
- Registered Dietitian.
- License to practice in New York State.
Preferred :
- Master's Degree preferred.
- Experience in individual and group counseling comprehensive diabetes self-management education and/or Medical Nutrition Therapy.
- 2-5 years administrative experience specializing in Diabetes program management and development, research experience, and quality assurance experience.
- Knowledge of current clinical guidelines and standards from the American Diabetes Association, American Association of Clinical Endocrinologists, and Association of Diabetes Care and Education Specialists.

- Meet Regulatory Requirements for pre employment screenings.
- Provide a copy of any required New York State license(s)/certificate(s).

Anticipated Pay Range:
The salary range (or hiring range) for this position is $91800 - $107100 / year.
The above salary range represents SBUH's good faith and reasonable estimate of the range of possible compensation at the time of posting. The specific salary offer will be based on the candidate's validated years of comparable experience. Any efforts to inflate or misrepresent experience are grounds for disqualification from the application process or termination of employment if hired.
Some positions offer annual supplemental pay such as:
- Location pay for UUP full-time positions ($4000)
