Based on clinical data and patients' lifestyle and culture, collaboratively develops a realistic plan with the patient or caretaker to improve eating to facilitate weight loss or prevent further weight gain. Serves as a nutrition resource for staff, students, interns, and appropriate community groups. May provide services in satellite health centers as arranged. Responsibilities Provides direct patient care as a registered dietitian. Provides nutritional assessment based on the patient's medical records and nutritional history. Assesses eligibility for the CYW 10-week weight management group program in collaboration with a multi-disciplinary team. Facilitates CYW weight management and parent/caregiver workshops and/or develops individualized eating plans for patients referred by General Pediatrics medical staff for nutritional consultation. Provides patient-centered, compassionate counseling to patients and, if necessary, family members and caregivers, including the planning, purchasing, and preparation of foods to meet the patient's individual needs. It is currently available in Somerville, Cambridge, Malden, Allston, Arlington, Brighton, Chelsea, Everett, and Medford. The Elder Service Plan of Cambridge Health Alliance operates under a contract with the Massachusetts Departments of Medical Assistance and the Centers of Medicare and Medicaid Services. The Registered Dietitian position with Elder Service Plan will be responsible for the following: 1.Responsible for initial and ongoinag assessment and counseling of participants. 2.
