This is a 16 hour per week position.Primary responsibilities include:*Completing comprehensive nutrition assessments for all residents and incorporating nutrition interventions into the individualized interdisciplinary plan of care for residents as indicated. *Insuring that all assessments and care plans meet professional standards of practice, reflect current evidence-based practice, and are in full compliance with all applicable regulatory statutes. *Participating with the interdisciplinary teams to provide professional guidance for assessing and implementing care strategies and monitoring nutrition and hydration status of residents.
