Job Title: Dietician Health Promotion and Disease Prevention Program Manager Company: Location: White River Junction, VT Description: This position is located in the White River Junction, Vermont Medical Center under the Supervision of the Primary Care Service Line Manager. The incumbent will work in conjunction with the Service Line Manager and Facility Leadership to oversee and coordinate the Health Promotion and Disease Prevention Program. The Incumbent will also serve as the primary facility point of contact for the Healthier US Veterans Initiative. Duties include but are not limited to: Advises facility and primary care leadership on clinical and administrative issues related to health promotion and disease prevention, including clinical preventative services Is responsible for strategic planning, program development and implementation, monitoring and evaluation of health promotion and disease prevention programs and services in the facility.
