DIABETES EDUCATOR Premier HealthNet is the region's largest Primary Care and Specialty Network serving communities in the greater Dayton and Southwest Ohio areas. We are currently seeking a Diabetes Educator to work as part of the support staff team for our diabetes specialty office in Dayton. The successful candidate will be a qualified and experienced Registered Nurse, BSN preferred. CDE will be an advantage. The office is typically open Monday to Friday, 8-5, and applicants must be available to cover vacation and sick days, and periods of high patient volume. Patient areas will include outpatient at the practice and inpatient at Miami Valley Hospital. Applicants must have the ability to asses the learning needs of individuals with diabetes and provide specific education with a focus on developing the patient's diabetes self-management skills. Job Title: Diabetes Educator Company: Cleveland Clinic Foundation Location: Cleveland, OH Description: DIABETES EDUCATOR - OUTPATIENT The Diabetes Educator provides educational resources to physicians, nursing personnel and patients. Assists with the overall coordination of the diabetes education program. Plans and provides one-on-one and/or group instruction for diabetes self management to persons with diabetes, their families and/or significant other based on individualized needs assessment. Minimum Qualifications: Bachelor of Science degree in Nursing or related field or the equivalent in training and experience. Must be self directed, highly motivated individual with ability for independent problem solving. Must have knowledge of age specific learning principles. Must be able to assess and interpret data about the patient's status in order to identify each patient's age specific needs and provide the care needed by the particular group. Minimum of three to five years experience in medical-surgical nursing or related field. Previous teaching experience preferred. Current RN licensure and registration in the State of Ohio. Must have completed 24 hours of basic diabetes education inclusive of diabetes-specific topics, educational principles and behavioral stategies. CDE preferred or CDE eligible within one year of hire. Requires full range of motion, manual and finger dexterity and eye-hand coordination. Requires standing and walking for extensive periods of time; corrected hearing and vision to normal range; some exposure to communicable disease or body fluids.
