They provide formal instruction/education regarding diabetes management to include medical nutrition therapy. They serve as a health resource to the community by participating in community events, providing screenings for diabetes detection and coordinating support group meetings. Imagine yourself Providing care following the National Standard for Diabetes Self-Management Education (DSME) as defined by the American Diabetes Association (ADA). Assessing and evaluating patient needs in developing plan of care. Updating care plan as needed. Under physician order, may adjust diabetes medications. Serving as an educator and resource to other caregivers regarding diabetes management. Maintaining communication with physicians and other health care providers regarding patient’s plan of care. Assessing the educational needs and learning barriers of the designated population. Integrating health promotion and preventive aspects of care. Providing formal instruction on diabetes management to the community. Serving as a health resource to the community by participating in community events, providing screenings for diabetes detection and coordinating support group meetings. Participating in the development and evaluation of patient education program and materials. Serving as a member of the Diabetes Advisory Board Committee. This position includes education at three Aurora Medical Group clinics in the Waukesha/Kettle-Moraine area. The hours and days of the week are flexible. General Qualifications Current certification as a Dietitian granted by the State of Wisconsin and current registration as a Registered Dietitian (RD) granted by the Commission on Dietetic Registration. Knowledge, skills and abilities required to perform this job are typically acquired through a minimum of 2 years of progressively responsible experience as a Dietitian with advanced clinical and technical knowledge of Diabetes and related conditions. Certification as a Certified Diabetic Education (CDE) granted by the National Certification Board for Diabetes Educators (NCBDE) required within 2 years of employment or transfer into this job. Excellent interpersonal and group communication skills. Maintains a high degree of professional competence by ongoing education and membership in professional organizations. Provides comprehensive diabetes education and care for patients with Type 1 DM, Type 2 DM, Gestational DM, IFG, Pre-Diabetes, etc in a clinic setting. License, Certification, Registration: Registered Nurse, Registered/Licensed Dietitian, or Registered Pharmacist. In addition to one of these registrations or licensures, preferred certification as a Certified Diabetes Educator (CDE) or eligibility to sit for CDE exam. Equipment/Tools: Work in a typical office, hospital, and village clinic setting. Comprehensive knowledge of PC computers. Ability to operate technology such as digital projector and digital camera. Ability to operate diabetes care medical equipment such as glucometer, ht./weight scale, etc. Requires travel to villages via small plane/sub-arctic conditions, moderate lifting of equipment, occasional long working hours. Specialized Knowledge and Skills: Strong knowledge base in the field of diabetes and public health. Cross cultural communication skills essential. Bi-lingual (Yup'ik/English) preferred.
